CMUNC requires the following liability forms to be filled out and submitted by April 1, 2024.

Please submit the Parental Consent Form (one per delegate/student) at this link. Please combine your entire delegations’ forms into one PDF, and follow the instructions laid out in the Google Form.

Please submit the Advisor Consent Form (one per advisor/adult/chaperone) at this link. Please submit one form per form submission, corresponding to the name of the person filled in, and follow the instructions laid out in the Google Form.
